2026 US HHS report attacking gender-affirming care

In August 2026, The Trump Administration US Department of Health and Human Services released a report attacking trans healthcare, which the authors call “sex-rejecting” procedures. It is titled “Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of ‘Gender Medicine.'”

All the contributors are prominent anti-trans organizations and activists, including Independent Women’s Forum, Center for Christian Virtue, and Ethics and Public Policy Center. Contributors Eithan Haim and Quentin Van Meter are both physicians who have been vocal opponents of trans healthcare.

Background

In May 2023, Eithan Haim, a general surgery resident in Texas, anonymously passed individually identifiable patient health information from the Texas Children’s Hospital gender clinic to anti-trans activist Christopher Rufo. Rufo accused the hospital of secretly continuing to provide transgender healthcare for young people despite having publicly claimed to have ended its program.

The following day, the Texas Legislature passed SB-14 banning such procedures statewide. In June 2024, federal prosecutors charged Haim with four counts of illegally obtaining protected health information. In January 2025, following a Trump executive order targeting what it called political weaponization of the DOJ, the case was dismissed with prejudice.

Since then, Haim has testified before Congress and appeared at events like Genspect’s Detrans Awareness Day 2026. One of Haim’s main claims is that providers have been billing trans healthcare under other medical codes to conceal the type of care provided.

Report summary

The report’s main thesis is that trans healthcare is a scandal that spread through a combination of financial incentives, ideological capture of medical institutions, and Biden-era federal policies.

Chapter 1 claims that gender clinics created a novel, high-margin, lifelong revenue stream for pediatric hospitals, generating about $120 million since 2019.

Chapter 2 outlines the alleged fraudulent medical coding and constitutes the bulk of the report.

Chapter 3 outlines Biden-era HHS, DOJ, EEOC, OPM, and Defense Department actions that promoted or mandated coverage or access to these procedures.

Chapter 4 accuses major medical and professional organizations, including WPATH, the Endocrine Society, and the American Academy of Pediatrics of bias in favor of trans healthcare, which the authors claim influenced how providers delivered this care.

Chapter 5 contains testimonials from eight ex-trans activists who claim they were harmed by the transition-related healthcare they requested and received:

The conclusion claims that “parents encountered wolves in white lab coats who pushed vulnerable children down a path towards irreversible harm from sex-rejecting procedures. Financial incentives from the medical industry and lobby; political and ideological pressures; and failure of oversight at the local, state, and federal levels have all contributed to the rapid medicalization of these procedures and resulted in long-term physical and psychological consequences for many patients.”

On the day it came out, Vice President J.D. Vance, HHS Secretary Robert F. Kennedy, Jr., and HHS Assistant Secretary Brian Christine announced they were “referring this matter to the Department of Justice to launch a federal criminal investigation.”

Response

The report was covered in the anti-trans press, with a few neutral sources reporting objectively on the report.
